TORONTO–(BUSINESS WIRE)–The Durham Ontario Health Team (OHT) and Community Care Durham (CCD), in collaboration with Health Espresso and with support from the OBIO® Life Sciences Critical Technologies & Commercialization (LSCTC) Centre of Excellence, are proud to announce the launch of a cutting-edge, digital community engagement tool. This strategic initiative marks a significant step forward in modernizing patient care, streamlining information access, and enhancing communication across the healthcare continuum.

Accessing and navigating health and social services remains a major barrier for many Ontarians, leading to fragmented care and poor patient outcomes. This partnership brings together Durham OHT’s commitment to community-based, integrated care and Health Espresso’s advanced digital health technology to provide patients with tools to navigate the right services for them.

Key objectives of the platform include:

  • Digital Access – Navigation of local health and social services to simplify and accelerate access to care.
  • Communication – Enabling seamless sharing of educational content and providing secure messaging between residents and their community health and social services partners, promoting better-informed, engaged patients.
  • Navigation – Helping residents navigate local services such as walk-in clinics, urgent care centers, and pharmacies.

The OBIO® LSCTC Centre of Excellence played a pivotal role in supporting this commercialization effort, helping to scale this critical technology and make it accessible to communities across Ontario. This community engagement tool will be rolled out in phases across Durham OHT member organizations, with ongoing evaluation and refinement based on patient and provider feedback.

The Durham Ontario Health Team (OHT) is a collaborative group of organizations and patient, family and care partner advisors working in partnership to improve access and delivery of coordinated health services. The group is comprised of partners from across the care continuum, including primary care providers, hospitals, mental health and addiction services, social support services, home and community care support services and long-term care.

​The Ministry of Health (MOH) announced the province’s first OHTs in December 2020. The Durham OHT was among this group.
